Visit Expectations

Preparation

  1. Check your insurance for eligibility, provider network, co-payment, or co-insurance.

  2. Set up your portal account via the invitation provided by our office.

  3. Complete the forms in your portal at least 48 hours in advance.

  4. To reschedule your appointment, please provide 48 hours notice.

  5. Allow two hours in your schedule for the first visit; bring your insurance card and photo ID.

  6. When you arrive in the parking lot for your appointment, check in via your portal and make payment for your visit.

o This practice does not submit out-of-network claims or facilitate single-case agreements.

o Payment is due at the time of service. We cannot carry account balances or payment plans.
